Hope 4 The Holidays Registration

This form is for registration for the Hope For The Holidays giveaway on Saturday December 13, 2025. The giveaway is for residents of Kentucky and individuals will be required to show ID to verify residency. Please fill out all required fields to ensure proper submission of your registration. All information must be able to be verified. Please only fill out information that pertains to you and your household only. You cannot fill out the form for another household or individual. The Application deadline is November 8, 2025 at 5:00PM. NO applications will be accepted after this date.
